I authorize Medi-Serv to charge my credit/debit card for this one-time payment of my invoice(s) referenced in this form. In the event Seed Technologies is unable to secure funds from my credit/debit card for this transaction for any reason, including but not limited to, insufficient funds in my credit/debit card account or insufficient or inaccurate information provided by me when I submit my electronic payment, further collection action may be undertaken by Seed Technologies to the extent permitted by law.

Refund Policy

Upon the successful transmission and processing of this payment, I understand that this payment is for services already rendered and that Medi-Serv will not be issuing a refund for this transaction.

We will save your information for billing purposes only. However we do not save your full credit card number nor release your information to third parties.
